Provide high quality clinical services to individuals referred for substance treatment, including short-term evidence-based substance treatment interventions and support services. •Consult with providers as part of the interdisciplinary team and generate recommendations, referrals, and case management as needed. •Conduct biopsychosocial evaluation for patients referred for substance use assessments including evaluations for individuals referred for medication assisted therapy (MAT) for opioid use disorder and alcohol Use disorder. Evaluation will include a comprehensive psychosocial history, ASAM placement criteria, readiness for change, and brief, validated assessment measures to evaluate trauma history, mood, and perceived level of functioning. •Provide health behavior change interventions for patients with substance use disorders using Motivational Interviewing. •Conduct safety and risk assessment as needed. •Generate documentation of assessment and intervention in the medical record in a timely manner. •Actively participate in regular team meetings and huddles of the interdisciplinary team at primary care clinic. •Participate in substance treatment counselor group supervision and administrative meetings to maintain cohesion and improve awareness of department functioning. •Participates or provides leadership of department, agency, or community meetings as well as internal Denver Health workgroups. •Provide teaching and presentation of relevant topics to the medical residents and training programs within Denver Health and University of Colorado. •Provide services in accordance with evidence-based clinical framework and is knowledgeable about effective practices in primary care medical settings. Possesses knowledge and understanding of the disease model of addiction, substance use disorders, psychiatric and physical health conditions. •Ability to maintain professional boundaries and confidentiality of patients\' records
