As Health Equity & Engagement Manager, I lead organizational efforts to strengthen community relationships, advance equitable access to care, and expand awareness of Bluegrass Care Navigators’ mission across the Commonwealth of Kentucky. My role bridges healthcare, community engagement, and strategic partnership development to ensure patients and families receive compassionate, accessible care regardless of background or circumstance.Key Responsibilities-Develop and implement health equity engagement strategies that expand access and reduce barriers to care in underserved communities-Lead community outreach initiatives, including faith-based collaborations, senior programs, and grassroots partnerships across Kentucky-Design and facilitate educational programs and presentations for staff, community leaders, and partner organizations to increase awareness and trust in hospice, palliative, and serious illness care-Build and manage strategic partnerships with faith communities, civic organizations, educational institutions, and healthcare providers to extend BCN’s reach and impact-Track and report on engagement metrics and outcomes through dashboards and evaluations to measure progress and guide decision-making-Represent Bluegrass Care Navigators at community events, conferences, and leadership forums, serving as an advocate for inclusion, equity, and accessibility in healthcare delivery.
