Provide support services to resource families, including weekly and/or bi-weekly home visits, consultation and counseling, crisis intervention, referral and coordination of community services- Assess the needs of foster child(ren) and ensure that the appropriate range of clinical and concrete services are accessed as needed for each child- Monitor resource parents’ activities to ensure compliance with all program standards and polices, and ensure that all required documentation is submitted by resource parents in a timely manner- Collaborate with County Social Worker and other team members to coordinate services for foster children and their resource families, including counseling, medical, and dental care, Child and Family Team meetings, and birth family visits- Facilitate regular Child and Family Team meetings to assess for minors’ needs and goals, and to ensure regular communication amongst child’s providers and resource parents
