Vital CxNs was founded to co-create a more accessible, effective, and equitable health ecosystem by convening cross-sector stakeholders, leveraging critical resources, and investing in data-driven, locally sustained community health infrastructure. Our goal is to tackle Boston\'s persistent and pervasive health inequities through facilitating collaboration, enhancing efficiencies, and discovering synergies amongst existing community health resources and stakeholders. We are a team of community organizers, medical professionals, and public health practitioners, bridging gaps and forging partnerships between health and wellbeing, healthcare and community, and individuals and institutions.
