About Annette Dubard

Dr. DuBard is a Senior Advisor for Aledade, a company committed to helping independent primary care practices and their patients thrive under new models of care delivery, achieving better health at lower costs. She previously served as Aledade\'s North Carolina Market President, extending Aledade\'s support model to 240 independent practices with over lives under management in successful value based arrangements with Medicare, Medicare Advantage, commercial and Medicaid payers. Prior to joining Aledade, she was the Chief Health Information Officer and SVP for Population Health Analytics for Community Care of North Carolina, overseeing data strategy and information systems used statewide to facilitate care coordination and quality improvement for Medicaid beneficiaries. Her work in transitional care, predicting impactability, and optimizing return on investment for community-based care management was nationally recognized with the 2016 inaugural Hearst Health Prize in Population Health. She received her BA from Rhodes College and her medical degree with honors from the Johns Hopkins University School of Medicine; and completed family medicine residency, preventive medicine residency, and a MPH in Health Policy and Administration from the University of North Carolina Chapel Hill. Her experience includes several years of community health center primary care practice, and she continues to provide office-based addiction treatment with Piedmont Health Services. She remains affiliated with UNC as a Fellow of the Cecil G. Sheps Center for Health Services Research and Adjunct Associate Professor of Family Medicine, with research interests in underserved populations, delivery system reform, quality improvement, and management of high risk patients. Her work has been published in a variety of venues including JAMA, Health Affairs, Medical Care, American Journal of Public Health, Population Health Management, and the American Journal of Preventive Medicine.
