   AdvantageCare Physicians is the largest physician-led multi-specialty medical practice in the New York metropolitan region, comprised of over 400 primary care physicians and specialists who serve nearly patients annually across 36 locations. AdvantageCare Physicians was created in 2013 to improve population health across the New York Metro area by delivering high quality, convenient and cost efficient care to a largely working class, traditionally underserved and culturally diverse patient base. AdvantageCare Physicians medical offices are located where our patients live and work throughout Manhattan, Brooklyn, Staten Island, Queens and Long Island with convenient office hours, including evenings and weekends. AdvantageCare Physicians promotes successful population health management with a unique delivery system that integrates clinical services across providers, settings of care, conditions and time. We transformed each of the primary care physician-led care teams into patient-centered medical homes (PCMHs) obtaining National Committee for Quality Assurance (NCQA) Level recognition at all 36 locations. AdvantageCare Physicians is connected to our communities. We offer care at each location that is culturally sensitive and supported by providers and care teams who speak the major languages in each neighborhood. AdvantageCare Physicians is committed to creating healthier lifestyles for New Yorkers through preventive medicine, education and community outreach.
