Funded by the Los Angeles County Department of Public Health, Division of STD & HIV Services, the Medical Care Coordination (MCC) Program focuses on improving health outcomes for individuals living with HIV who have complex medical conditions. The program integrates clinical outpatient care with supportive services to address both medical and social determinants of health.Working as part of an interdisciplinary team, the MCC Patient Care Manager (PCM) ensures comprehensive and thorough assessments of patients’ psychosocial needs, particularly those related to behavioral health. The PCM primarily conducts brief interventions aimed at improving a patient’s health status in areas such as medication adherence, addiction, mental health distress, risk reduction, and disclosure intervention. The PCM\'s care and support responsibilities include-Overseeing all psychosocial aspects of clients’ treatment in the Medical Care Coordination (MCC) program-Utilizing training in motivational interviewing and applying the Stages of Change/Transtheoretical Model to conduct comprehensive psychosocial assessments and implement appropriate interventions based on patients’ readiness to change-Facilitating case conferences with MCC and clinic staff, including medical providers, to streamline care coordination and address barriers to care, thereby improving health outcomes-Collaborating with the Medical Care Manager (RN) and Case Manager to develop comprehensive care plans for clients-Coordinating necessary documentation for public and private assistance programs-Maintaining complete and accurate client files, documenting services provided using all required documentation methods.
