I’m Angela Jacob, a SARTAC Fellow and independent provider of disability case management, resource navigation, and consulting services. My passion is helping individuals with disabilities and their families access the resources, benefits, and opportunities they deserve — without being overwhelmed by red tape. Through my work, I provide: One-on-one navigation & case management to guide individuals through complex systems Consulting support for organizations and nonprofits, including: • Identifying unique disability resources for the people they serve • Strengthening fundraising efforts to expand impact • Improving membership engagement and communication • Providing navigation assistance to clients and families needing extra support Advocacy and resource identification to ensure no one falls through the cracks I bring both professional expertise and lived experience, giving me a consumer-focused perspective that helps me connect with individuals, families, and organizations in a meaningful way.
