My honorarium within Sandwell works directly with the TVN team to implement and validate a leg ulcer pathway redesign program, This includes 4 key work streams focusing on patient concordance, documentation, clinical best practice and education. This project will stay close to the National Wound Care Strategy Program and lead the roll out of education and competency training across Sandwell and West Birmingham Hospitals NHS Trust.This includes:# Empower staff to deliver best in class patient care for lower limb management through a train the trainer programme # All sites to have adequate training coverage and training plan within 6 months# Work with MDT to support Lower Limb Treatment and long term Management # Use diverse skill set of relevant MDTs – Dermatology, Podiatry, Vascular & GP Services# Improve referral relevance & reduce waiting times for patients with a lower limb wound# Formalise SystmOne templates and the documentation for Lower Limb Treatment and Management# Further develop data capture plan from 2018-20 project initiation# Improve current data accuracy through implementation of a new documentation module and algorithm # Improve patient care and clinical / financial outcomes in Lower Limb Management# Develop and support roll out of a patient passport# Align Nursing Teams with the enhanced service through a online portal housing educational conteent, pathways, guidelines, proceedures and competencey frameworksKPI: Ensure all patients with a lower limb wound enter the clinical pathway before 4 weeks
