Promote self-determination to each patient/client to gain confidence and reassurance, as well as to advocate towards my clients. Provide an array of services for my clients, not limited to, temporary respite stays, to short-term rehabilitation, to long-term skilled nursing care, as well as other specialty programs and services (i.e, in-house dialysis, pain mgmt, PT/OT/ST, cardiac care).Participating in multidisciplinary team meetings (MDT), maintaining mutual communication amongst other insurances and community outreach to meet the needs of my clients. Assist and complete high-risk concerns, complaints, and problem-solving issues. Identifying and investigate underlying issues and concerns of residents/patients. Coordinated follow ups and discharge planning for patients and their families. Assist with guardianship. Provide guidance and training for interns/volunteers. Remain transparent and adaptive towards complying to rules and guidelines of the agency. Consistent monitoring, record keeping, and professionalism throughout. I was able to navigate and officially complete AHCA reviews annually and drop-ins.With supporting documentation and consistent observation, I successfully won two (2) 30-day discharges as per Department of Children and Family: Appeal Officer. Successfully received no red/flagged items in Social Service Department during Joint Commission for 2025.
