Engage in assessment and treatment in the areas of cognition, swallowing, voice, speech & language, and executive functioning in adult/ geriatric patients recovering from CVA, TBI/ ABI or encephalopathy• Tailor and summarize assessment results for various audiences including patient, family, and members of an interdisciplinary team• Develop and implement plan of care with goals tailored to the continuum of care established and patient’s needs• Integrate evidence-based practices in treatment sessions including Spaced Retrieval, Response Elaboration Training (RET), Semantic Feature Analysis (SFA), and Melodic Intonation Therapy (MIT)• Create low-tech communication boards based on patient’s needs and wants observed throughout sessions as well as encourage implementation through modeling and providing opportunities for practice• Complete thorough chart reviews to identify risks for dysphagia, nutritional compromise, risks for pneumonia, and medical conditions pertinent to dysphagia• Perform and interpret data from Oral Mechanism Examinations, Bedside Swallow Evaluations (BSE), and Modified Barium Swallow Studies (MBSS)• Identify signs and reasons to recommend an MBSS/ FEES as well as make appropriate (i.e, nutrition, ENT, GI)• Track patient data independently and wrote daily progress notes, evaluation reports, and discharge reports• Collaborate with physicians and the interdisciplinary team during weekly patient care conferences by providing updated patient progress and goal revisions when appropriate • Re-evaluate, modify treatment plan and goals, and participate in discharge planning• Provide caregiver education and training in the areas of communication, cognition, and swallowing tailored to the patient’s needs
