Coordinate Health and Human Service Linkages; providing information and referrals to resources within the school or mental health resources within the community. Provide Case Management services – to address the needs of students referred for a variety of mental health, emotional, developmental, behavioral and/or social problems. Assist students with the evaluation and application to transfer to Four Year Colleges/Universities to continue their education. Health And Human Service Linkages; community outreach and collaborate with mental health and/or social service partners. Collaborate with Project Manager at school site to coordinate service delivery. Confer with the Project Manager – monthly progress reports, consultations and case staffing. Documentation of service delivery contacts. Maintain case folders for targeted students. Attend Cluster Meetings and CIS In-Service/Trainings. Attend CIS Program Team Meetings
