Early Childhood Health and Wellness ProgramProvide technical assistance to Berkeley\'s pediatric medical providers enrolled with First 5\'s Help Me Grow program in order to increase developmental and autism screening and referrals. Promote Help Me Grow internally to public health programs and to families seeking support with developmental concerns for children aged 0-5. Conduct workshops on topics related to early childhood development and health for underserved families participating in various other public health programs within the City of Berkeley. Provide consultation and support for the City of Berkeley\'s public health programs serving vulnerable families at the West Berkeley Family Wellness Center.Services to Enhance Early Development (SEED)Nursing case manager for Alameda County\'s SEED program, which provides intensive relationship-based services for the youngest children who are in out of home care, their birth parents, and their caregivers in the child welfare system with the hope of achieving timely permanence, either through reunification, adoption, or legal guardianship. Collaborate with dyadic therapists, child welfare workers, and parent advocates to offer comprehensive trauma-informed services to children and families in the child welfare system. Screen and identified children in need of specialty medical and developmental services. Coordinate with primary care and specialty clinicians, Regional Center of the East Bay, local agencies, Medi-Cal, and families to ensure health care access and timely follow-up. Provide in-home support and education to parents and caregivers to meet the health and developmental needs of children ages 0-5 in foster care.
