Accomplishments include:* Created the \"Serious Illness Scorecard\" rating each state on their capabilities to meet the unique needs of their residents with serious illness* Created and led 3 \"Learning Communities\"(for MA plans, for ACOs, and for PACE programs) to teach patient stratification and home-based care engagement strategies.* Developed and led the \"CAPC Payment Accelerator,\" teaching palliative care programs how to succeed under value-based payment. Held 6 workshops, teaching hundreds of clinicians.* Ongoing analysis and advocacy on CMS regulations and payment models, including Primary Care First, Direct Contracting, and oncology bundled payment programs.* Develop tools and technical assistance to support optimal payment and efficiency for palliative care programs, including the use of telehealth.
