As a Health Advocate, I provide basic needs case management within a defined scope to Legacy patients. I focus on addressing social determinants of health (SDOH), connecting clients to community resources, and collaborating with healthcare providers to support whole-person care.•Screen and assess clients for needs related to social determinants of health, including housing, food, transportation, and financial assistance.•Conduct real-time research to identify and refer clients to appropriate community-based services using all available tools to ensure the best possible support.•Develop service plans in collaboration with clients and their families, tailoring referrals to meet individual circumstances.•Maintain timely and professional follow-up to confirm service linkage and address any barriers, using accessible, culturally sensitive communication.•Advocate with and on behalf of clients to reduce gaps in service and remove access barriers.•Participate in weekly Advocate Development Sessions to evaluate and enhance the quality and effectiveness of services.•Document all client interactions and service efforts in Epic (electronic health record system) within 72 hours to ensure coordination with the clinical care team.•Foster a respectful and inclusive environment for all patients, families, staff, and interns.•Collaborate with the healthcare team in a primary care medical home setting to promote timely access to care, continuity, culturally and linguistically appropriate support, and patient self-management.
