About Alexis Kerr

Graduating from IU School of Social Work in August 2023. Looking to work with the dual diagnosis population in mental health who have an intellectual or developmental disability, as well as a co-occurring mental health disorder. January 2022 - Employee of the Month for Sandra Eskenazi Mental Health Center. Completed Goal: Create a social group for my clients to learn to create meaningful relationships with others and learn how to navigate relationships of all kinds, while also learning skills necessary to have healthy relationships. This has been consistently attended and is full for about one year now (February 2023). Working on: Master\'s program for social work at IUSSW, graduating in August 2023. Completing my final practicum with Circle City Clubhouse, learning an entirely ‘new-to-me’ intervention strategy for those with mental health diagnoses and following the empowerment theory. Completed my first internship in Summer 2022 with WellSpring, HopeSpring, & The Community Service Center of Morgan County. I strive to influence my environment by being positive and building others up. I truly believe in setting an example for others in all that I do. I have earned my Bachelor\'s degree, majoring in psychology, minoring in communications with a human resources certificate from IUPUI. Things I enjoy doing: reading, learning, creating things, art, organizing, traveling, having deep and meaningful conversations, hiking, playing with animals and children, cooking/baking, and making people laugh! I pride myself on being attentive to all relevant details and understanding the requirements of any job i perform. I consistently strive for excellence, and look for new ways to build up my team and tackle daily challenges. Creative thinking and problem solving are also specialties of mine, as well as building relationships and rapport with others.    As a Care Coordinator, my job is to formulate, develop, and revise treatment plans, BPSA\'s, ANSA and CANS assessments, and other necessary client assessments. I advocate for clients to ensure access to needed services are known and attainable, as well as build relationships with community teams and resources to better serve my clients. Identifies any high-risk patient symptoms or circumstances that would result in risk for self-harm or harm to others and notifies appropriate higher level clinical staff for consultation. Completes appropriate legal documentation as indicated on treatment plan. Interprets and applies service and billing procedures according to client needs; completes billing according to guidelines for timeliness and accuracy. Works with family, significant others and community supports to assist clients in meeting goals. Alters activities and behaviors to reflect and ensure adequate care appropriate to the age of clients served. Provides quality case management and activities of daily living skills training to clients. Monitors client progress and reflects this in documentation.
