   Manager, Skin Health Team Leader for a 148-bed facility for long term and short-term care. As the wound care coordinator, I have closed 90% of the wounds in my facility. Currently working with a consulting group twice a week and assisting with wound care at other Genesis facilities. Work in conjunction with other disciplinary team members to coordinate appropriate care for patients in the facility, Including but not limited to, ordering Low Air Loss Mattresses and Wound Vacs, as well as determining appropriate alternatives. Attend Care plan and Utilization Management once a week with other team members. Utilizing SWFT software, photographic program for measurements and documentation, comparing the progression of wound healing. Documentation in PCC.
