ABOUT ALBERT CHAFFIN
As a Medical Director at Legacy Health Partners, I lead a physician-led, clinically integrated network that leverages information technology and data integration to improve population health. I also serve as the Medical Director of Inpatient and Outpatient Telemedicine, working closely with implementation, analytics, and technology teams to ensure reliable, state-of-the-art access to Telehealth. This role integrates seamlessly with my work in population health and value-based care by leveraging innovative telemedicine solutions such as remote patient monitoring, tele-sitting, tele-nursing, and virtual on-demand ambulatory care. These technologies enhance care delivery, improve operational efficiency, and reduce barriers to access, supporting the delivery of high-quality, cost-effective care while optimizing patient outcomes.In addition to my roles at Legacy Health Partners, I am a Consultant Pediatrician at the Randall Children’s Gender Clinic, where I provide direct patient care, including prescribing gender-affirming medical treatments such as hormone therapy. I collaborate with a multidisciplinary team to offer comprehensive services, including mental health support, STI screening, and PReP access. In partnership with pediatric endocrinologists, I work to address gaps in primary care and ensure holistic, affirming care for youth. Additionally, I contribute to the development of clinic protocols and care pathways aimed at improving outcomes for gender-diverse patients.
