Analyze patient encounters listed on weekly DNFB reports for missing provider operative reports, physician orders, procedure notes and other documentation such as H & Ps and discharge summaries. Initiate and follow-up on action to produce these documents and pass to coders. Patient types and locations include inpatient, outpatient ancillary, outpatient surgery, ED, and provider orders from all 5 Northside hospitals and numerous Northside physician groups. Utilize Cerner and Allscripts STAR to analyze encounters and charges.• Resolve all revenue cycle, release of information, and patient identity issues from across the Northside system.• Remove and move documentation as needed.• Investigate incomplete charts identified through release of information requests. Work with MRO staff.• Authenticate documents not needing physician signatures and issue physician document and signature deficiencies.
