Family Resource Coordinator - Children\'s Village @Virginia Mason Memorial

Provide support to families through advocacy by assisting in identifying concerns priorities and resources, anticipating informational needs, assisting with completion of appropriate application forms, and assessing comprehension and engagement.Describe programs clearly to the parents of eligible children by participating in developmental evaluations to determine eligibility in appropriate programs, developing a plan with the family to coordinate community resource to meet the family’s needs, and coordinating referrals to local school districts. Monitor individual plans on an ongoing basis including reviewing progress toward outcomes, objectively and accurately identifying potential/real barriers, linking families to appropriate community resources, amending service plans as needed to reflect changes in frequency, setting, or duration. Gather information from all sources to confirm the family’s needs are being appropriately addressed, including review of the medical record, communication with the team of providers and input from the family.Facilitates collaborative team meetings and case conferences as well as works with multi-disciplinary team to provide family-centered services at Children’s Village.
