    The TOFOS Case Manager will attend all scheduled Twin Oaks Court hearings and shall provide notice to the TOFOS Director in advance of an absence. Personnel shall be responsible to ensure that a written notice of the client’s status is provided to the committing Court for every review hearing. Additionally, the Case Manager will give a verbal report directly to the personnel at these hearings.The TOFOS Case Manager shall serve as the liaison between the Florida Juvenile Court personnel, the Florida Department of Children & Families (DCF), the Florida Agency for Persons with Disabilities (APD), the Florida Department of Juvenile Justice (DJJ), and Twin Oaks Independent Contractors to include Competency Trainers and Forensic Evaluators. The Case Manager shall at a minimum make monthly contact with all entities listed above to provide updates in the provision of competency restoration services.The TOFOS Case Manager will conduct face to face contacts with each regionally assigned client at least once monthly. The client’s Parent and/or Guardian are required to be present and shall meet with the Case Manager to review the provision of services performed by Twin Oaks. This monthly contact with the client and their Parent and/or Guardian shall include a validation of services reported by subcontracted Competency Trainers.The TOFOS Case Manager once a month shall assess the client’s prognosis in being restored to competency and the findings of this review shall be documented in the Case Management Monthly Assessment.The TOFOS Case Manager will perform a monthly review the Competency Restoration Service Plan to determine the needs in the client being restored of competency. The Case Manager will provide assistance in the coordination of referrals to identified service needs. The TOFOS Case Manager shall observe client’s environment both at home and at school during the monthly contact visits.
