Worked closely with all providers, RN’s, Case Managers, Health Navigator Coordinators, & Enrollment Coordinators in providing patients with HIV/AIDS or at risk for HIV/AIDS education, resource referrals, follow-up, and adherence support• Conducted face-to-face assessment and re-assessment (including assessment of adherence to treatment, adherence to appointments, and family planning needs). • Monitored clients to assess the implementation and efficacy of the care plan and to ensure continuity of care. • Reviewed and monitoring CD4 and viral load lab values and making sure the most current lab values are recorded in the client file/database. • Documented client contacts in the case notes, on the required forms, and in the required database(s)• Facilitated linkage and re-engagement activities for clients who appear to be out of care• Educated clients about HIV and its transmission and complications and about HIV risk reduction.
