I play a key role in supporting patients with long-term respiratory and cardiac conditions to regain independence and enhance their quality of life. My primary responsibilities include assisting with rehabilitation programs in different Hospitals/locations and delivering both clinical and administrative support to ensure the effective operation of the Cardiorespiratory Rehabilitation Service.Core Responsibilities:Exercise and Rehabilitation Support: Facilitate and supervise exercise programs, working with patients in hospital-based gyms, community settings, and patient homes to ensure safe and effective rehabilitation. Additionally, assist the lead Physiotherapist in cardiac, pulmonary, and heart failure rehabilitation groups to support each patient’s unique recovery journey.Patient Assessment and Safety: Collaborate with team members to assess patients’ abilities, identify potential risks, and provide appropriate equipment. Conduct safety checks on all equipment to ensure safe use during exercise and rehabilitation activities.Patient Independence and Progression: Work closely with the clinical team to support patients in their journey to return to previous levels of independence, offering both physical and motivational assistance to encourage their progress.Administrative Duties: Perform essential administrative tasks, including data entry, and handling phone messages. Effectively use IT to manage patient records and support the digital infrastructure of the rehabilitation service.Key Skills and Attributes:Organisation and Self-Motivation: Demonstrate strong organisational skills and a proactive approach to both patient care and administrative duties, maintaining a smooth workflow within the rehabilitation team.Interpersonal Skills: Engage with patients, team members, and healthcare staff effectively, fostering a supportive and empathetic environment that promotes patient comfort and trust.

The results of the consultation impacted the community and aided Barratt London\'s development plans.I consistently seek opportunities to gain insights from my peers. I strongly advocate for maintaining a balance between work and personal life, emphasising the importance of self-care.
