TellyHealthMD is a physician-led care delivery organization designed to manage high-risk patients outside traditional clinical settings, where deterioration most often begins and visibility is lowest. This model addresses the portion of the care continuum responsible for a disproportionate share of avoidable utilization, quality deterioration, and member disengagement. TellyHealthMD deploys physicians and advanced practitioners to deliver in-home and virtual evaluations, preventive services, diagnostics, chronic disease oversight, remote monitoring, medication management, urgent triage, and reassessments aligned with regulatory and plan requirements. Our work focuses on high-acuity populations across Medicare Advantage, Medicaid, MLTC, MAP, D-SNP, provider organizations, and value-based programs — particularly individuals with limited mobility, complex comorbidities, caregiver dependence, or inconsistent engagement with traditional systems.
